Answer: No ID cards are issued for the Dental Plan. You do not need a card to receive service. To receive services, give your dentist the Dental group and sub-group number for the dental plan option in which you are enrolled and your 8-digit UMID number with an additional "0". The Dental Group Numbers are 5970-0001 for Option 1, 5970-0002 for Option 2, and 5970-0003 for Option 3.
