Answer: There are several places you can find a list of providers:

The Benefits Office website or the vendor website.

You can also call the HR/Payroll Service Center at 734-615-2000 or 866-647-7657 (toll free for off-campus long-distance calling within the U.S.) and request a provider book.

If you want to know if your provider participates with a particular plan, contact your provider and ask.
